In 2005, the industry recorded one accident for every 268,703 flights. By 2025, that figure had improved dramatically to around one accident for every 760,000 flights.<\/p>\n\n\n\n<p>While that progress is of course positive, it also means that when an accident does happen, the investigation that follows really matters even more than before.<\/p>\n\n\n        <div class=\"flex flex-wrap items-center justify-start -mx-2 mt-6\">\n                                    <a href=\"https:\/\/aerospaceglobalnews.com\/news\/iata-2025-safety-report-global-aviation-accidents\/\"  class=\"btn-ocean  mx-2 my-2  flex items-center flex-row \">\n                                        <span class=\"sr-only\">Read more: Global aviation remains extremely safe, despite rise in fatalities &#8211;  The IATA 2025 Safety Report<\/span>\n                    <span>Read more: Global aviation remains extremely safe, despite rise in fatalities &#8211;  The IATA 2025 Safety Report<\/span>\n                <\/a>\n                        <\/div>\n        \n\n\n<h2 class=\"wp-block-heading heading-two\">More than a third of accident reports miss international standards<\/h2>\n\n\n\n<p>IATA says too many investigations are failing to meet the standards set out in <a href=\"https:\/\/elibrary.icao.int\/product\/229733%3F_gl%3D1*14gm6u4*_ga*MjIzOTQ5NTQ3LjE3Nzg0MzkzMzI.*_ga_992N3YDLBQ*czE3Nzg0MzkzMzIkbzEkZzAkdDE3Nzg0MzkzMzIkajYwJGwwJGgw\"><strong><span style=\"text-decoration: underline;\">Annex 13 of the Chicago Convention<\/span><\/strong><\/a> \u2013 the international framework governing aircraft accident investigations.<\/p>\n\n\n\n<p>Between 2019 and 2023, only 63% of accident investigations were completed in accordance with Annex 13 requirements. The remainder were delayed, incomplete, unpublished or failed to meet international reporting standards.<\/p>\n\n\n\n<iframe loading=\"lazy\" width=\"560\" height=\"315\" src=\"https:\/\/www.youtube.com\/embed\/-t7YYbkw_rU?si=NpiZiPZLa1xspX5R\" title=\"YouTube video player\" frameborder=\"0\" allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture; web-share\" referrerpolicy=\"strict-origin-when-cross-origin\" allowfullscreen><\/iframe>\n\n\n\n<p><br>For an industry built on data-sharing and collective learning, that is something of a growing concern.<\/p>\n\n\n\n<h2 class=\"wp-block-heading heading-two\">Why are accidents being misreported?<\/h2>\n\n\n\n<p>The reasons vary. Alongside thorough investigations, that data can help identify trends and risks far earlier than before.<\/p>\n\n\n\n<p>But that only works if investigations are completed properly and shared across the industry.<\/p>\n\n\n\n<h2 class=\"wp-block-heading heading-two\">IATA backs Annex 13 reforms and new safety tools<\/h2>\n\n\n\n<p>To improve completion rates, IATA is supporting a series of proposed amendments to Annex 13.<\/p>\n\n\n\n<p>One of the key changes would make it clearer that states can delegate investigations to another country or regional investigative body when local capability is limited.<\/p>\n\n\n\n<p>The proposals also reinforce the importance of unrestricted access to evidence and greater transparency throughout the investigation process.<\/p>\n\n\n\n<p>Importantly, the updated guidance emphasises clearer communication not only with regulators and <a href=\"https:\/\/aerospaceglobalnews.com\/tag\/airlines\/\"><strong><span style=\"text-decoration: underline;\">airlines<\/span><\/strong><\/a>, but also with victims\u2019 families and the public.<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"576\" src=\"https:\/\/aerospaceglobalnews.com\/wp-content\/uploads\/2025\/07\/plane_a350_0-1024x576.jpg\" alt=\"Delta Air Lines Airbus A350 in flight.\" class=\"wp-image-25708\" srcset=\"https:\/\/aerospaceglobalnews.com\/wp-content\/uploads\/2025\/07\/plane_a350_0-1024x576.jpg 1024w, https:\/\/aerospaceglobalnews.com\/wp-content\/uploads\/2025\/07\/plane_a350_0-300x169.jpg 300w, https:\/\/aerospaceglobalnews.com\/wp-content\/uploads\/2025\/07\/plane_a350_0-768x432.jpg 768w, https:\/\/aerospaceglobalnews.com\/wp-content\/uploads\/2025\/07\/plane_a350_0-640x360.jpg 640w, https:\/\/aerospaceglobalnews.com\/wp-content\/uploads\/2025\/07\/plane_a350_0.jpg 1920w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><figcaption class=\"wp-element-caption\">Photo: Delta Air Lines<\/figcaption><\/figure>\n\n\n\n<p>Alongside the regulatory reforms, IATA has worked with <a href=\"https:\/\/aerospaceglobalnews.com\/tag\/icao\/\"><strong><span style=\"text-decoration: underline;\">ICAO<\/span><\/strong><\/a> and the <a href=\"https:\/\/aerospaceglobalnews.com\/tag\/ifalpa\/\"><strong><span style=\"text-decoration: underline;\">International Federation of Air Line Pilots\u2019 Associations<\/span><\/strong><\/a> (IFALPA) on a simplified <a href=\"https:\/\/www.iata.org\/contentassets\/5e15614c2579458fa2c84403c7763914\/icao_annex-13_at_a_glance_infographic.pdf\"><strong><span style=\"text-decoration: underline;\">infographic<\/span><\/strong><\/a> (PDF) explaining how Annex 13 investigations should progress step-by-step.<\/p>\n\n\n\n<p>The aim is to reduce confusion among the wider group of organisations involved in accident investigations and help reports get completed faster.<\/p>\n\n\n\n<p>IATA has also set up a central database that pulls together safety recommendations from accident reports around the world. Instead of being spread across different countries and agencies, the information is now in one place, making it much easier to spot patterns and recurring risks across the industry.<\/p>\n\n\n\n<h2 class=\"wp-block-heading heading-two\">Every investigation carries greater weight<\/h2>\n\n\n\n<p>IATA accepts there is no single reason why so many reports fail to meet Annex 13 standards, and that there is no instant fix.<\/p>\n\n\n\n<p>But the message is clear: aviation\u2019s impressive safety record cannot become a reason for complacency.
